We want to hear from you!
We’re looking for parents, caregivers and siblings planning for the long-term care of a loved one, and disabled self-advocates planning for their future, to record video testimonials about their experience. If you’d like to submit a video, please follow the instructions below. Your video may be used as part of the film, supplementary “bonus content,” and/or other materials shared with screening audiences and our digital community. Please note, we are extremely appreciative of all submissions, and they are all valuable to the development and progress of this project, but we cannot guarantee your video(s) will be featured publicly.

Instructions
- Your videos must be 5 MINUTES or less. Make sure to leave a slight pause before you start talking and after you stop. Record separate videos for each topic you want to discuss.
- Don’t worry about saying everything perfectly — our goal is to get your honest thoughts.
- If more than one person would like to share, please record separately, one person per video.
How to Record Your Video
- Please record with your phone (not a webcam) in a vertical position.
- Make sure you’re in a quiet space.
- Try to find a spot with good lighting, like facing a window.
- Keep the camera level with your face so you’re looking straight at it, not up or down.
You can answer all or some of the questions below, or share your own thoughts.
Questions for Parents and Caregivers
- What are your plans for the long-term care and support of your loved one?
- What options are you considering? What options are available to you based on your location, financial resources, and the level of support needed?
- What barriers are you facing? What are the biggest hurdles?
- If you have a solution…what does that look like? How is it working for your family?
- What are your concerns or fears about your loved one’s long-term living situation? Do you feel that your future is uncertain?
- Has your loved one expressed preferences or concerns about their future and desired living situation?
- What makes you hopeful about the long-term plans for your loved one and your family? What do you feel positive about?
- What do you believe is the ideal long-term care scenario for your family?
Questions for Siblings
- Are you / do you plan to be involved in the long-term care of your sibling? Why or why not?
- Have you talked about it with your parents? What does that look like?
- How do you feel about the future? What are your concerns, fears, and hopes?
- What do you believe is the ideal long-term care scenario for you and your sibling?
- If you are already involved in the long-term care of your sibling…what does that look like? How is it working for you and your family?
Questions for Self-Advocates
- What are your current plans for your long-term living situation?
- What options are available to you based on your location, financial resources, and the level of support needed?
- Do you feel like you have good options? Why or why not?
- What are your concerns or fears about your future?
- What makes you hopeful or excited about your future?
- What do you believe is the ideal long-term living situation for you?
Submit Your Videos
Please email your video(s) to info@quietlifecreative.com. If the file is too large to email, you can upload it to a service like Google Drive, Dropbox or WeTransfer, and send a link by email. Please ensure the files are not compressed, so we receive the highest quality version.
